NURSE – Specialized Services (N-2425-0505)

This is a Contract position in Montreal, QC posted September 4, 2024.

SUMMARY OF THE POSITION
A person who is responsible for providing nursing care services according to the bio-psycho-social needs of users or groups under her/his care. She/he evaluates the health condition of users, as well as determines and implements nursing care and treatment plans. She/he provides nursing and medical care and treatment aimed at maintaining and restoring health, as well as preventing disease.

She/he plans, provides and evaluates teaching activities intended for the users, their relatives and groups of people. In addition, she/he participates in research activities aimed at promoting health and preventing disease.

SPECIFIC FUNCTIONS
The nurse is responsible to coordinate, organize and prepare the telehealth or on-site appointments of the specialists and assist them during their visits. Afterwards, the nurse is in charge to review the medical records according to the report and ensure the proper follow-ups as required. She/he is responsible to review the medical consultations and/or reports of the beneficiaries entrusted to her/him temporarily. She/he will act as a link between the beneficiary, the communities and the specialists. She/he will gather and forward the pertinent information to the appropriate professionals in the Cree communities.

SPECIFIC FUNCTIONS
Patient evaluation* Act as a liaison with other health care professionals (social worker, nutritionist, physiotherapist, and also with other partners in the health community) to adequately meet the needs of the patient;* Monitor, evaluate and update interventions between specialist appointments in collaboration with the CMC.

Specialist support* Prioritize and organize the list of waiting patients based on clinical judgment, doctor's notes, test results, and follow-up with the patient;* Ensures that patient-related interventions are well documented and that all follow-up appointments are reflected in the clinical information systems of the Cree Board of Health and Social Services of James Bay (CBHSSJB);* Monitoring of time limits for pending consultations and follow-up requests and ensuring follow-ups with affected individuals as needed;* Participates in the preparation of files related to appointments with specialists (ensures that the prerequisites are made, communicates with the involved stakeholders if necessary);* Assists the specialist according to predefined needs or any other clinical need arising during a visit or telehealth clinic.

Patient and family support* Disseminates appropriate and accurate information to support informed decisions;* Ensures and maintains adequate and effective communication between the patient, the specialist and the CMC team;* Delegates certain aspects of care to other health professionals and/or the CMC team.

Collaboration and coordination of care* Communicates the care plan to the CMC Care Team (closing the post-visit record, updating the information in the CBHSSJB clinical information systems);* In partnership with the specialized services team and the CMC team, facilitates the coordination of nursing interventions for the patient and his / her family;* Refer patients to other healthcare professionals and the CMC team as needed.

Education:

  • Member of the Order of Quebec Nurses (O.I.I.Q.);
  • DEC in Nursing.

Experience:

  • Two (2) years of experience as a nurse.

Language:

  • Fluent in English;
  • Fluent in Cree or French is an asset.

Other:

  • May be required to work overtime occasionally depending on specialist visiting hours;
  • Will be required to travel on a regular basis in the communities of the Chisasibi Pole.
